Dr. James K. Kramer and his team are incredibly grateful that you’ve considered them for all of your oral care needs! As you get acquainted with our dental office and services, we hope that you take the time to learn how to prepare for your first visit. Below, you’ll find everything you need to get started, including expectations for your arrival, downloadable new patient forms, and how to best finance your treatments. Thank you in advance for taking the time to fill out these forms.
When you first arrive, one of our team members will greet you with a friendly smile on time; the last thing we want to do is keep you waiting. After getting your new patient forms, we’ll bring you back and complete an examination of your head, neck, TMJ, and mouth. Once Dr. Kramer completes the exam we will schedule you for your next appointment. In the meantime then takes all this information and determines any problem areas and formulates mandatory individual treatment options for you, he then personally goes over with you to educate you on your options so you can decide what’s the best alternative for you.
New patient forms are designed to give us as much information as possible about the current state of your oral and overall health prior to your first exam. The more info you provide, the better we can establish a treatment plan that addresses your needs. To save time during the check-in process, please download, print, and fill out your new patient form before arriving.
When you have dental insurance, which approximately 50% of our patients do, whether it’s through your employer or from the marketplace, it can make your basic treatments like cleanings all the more affordable. While we are not in-network with any plans, we will gladly file claims on your behalf. The best way to learn more about coverage for your dental insurance plan is to call our dental office directly. There, our team will walk you through what you can expect in terms of coverage.
Do you not have dental insurance? Do you still have unpaid costs despite having a dental plan? With the help of third-party financing, you can pay for treatments in more manageable chunks without breaking the bank in the process. CareCredit is just one of those services that make dental treatments, including those centered around cosmetics, affordable for far more people.
